Openstack Adatper fails to find Stack Name and creates duplicate stack with address conflict

During longevity testing there were be a sporadic error that a vf module would fail due to duplicate IP addresses. Examining logs it looks like the vfmodule stack create is sent but on a query to openstack Heat the stack is not found causing a duplicate stack create to be sent. The duplicate fails in heat validation due to duplicate IP address and a rollback occurs.
