Terminated PDPs are not being removed by PAP

When various PDPs were restarted, the former PDPs are not removed from the DB.  Consequently, the policy deployment report never shows new policies to be fully deployed.

Further investigation revealed that a DB connectivity failure occurred which may have thrown a RuntimeException, thus terminating the timer thread that PAP uses to check for expired PDP records.
